The pilot worked. The demo was impressive. Leadership nodded. Then nothing happened. This is the most common AI story in enterprise right now, and the reason is rarely that the model underperformed.

The RAND Corporation puts the failure rate for AI pilots at around 80%, nearly double that of traditional IT projects. That comparison matters. The technology stack in a typical AI pilot isn't more fragile than in a CRM rollout. What's different is that AI asks more of the people around it. It asks them to change their judgement, their workflows, their habits. That's a different kind of ask, and most organisations aren't set up to support it.

The Pilot Is a Success. The Implementation Is the Problem.

Pilots are, by design, insulated from the organisation. A small team, a controlled use case, a patient sponsor. They're built to succeed in a bubble. The problem is that moving to production means popping that bubble, and most organisations treat that transition as a technical handoff rather than an organisational change project.

BCG's analysis of AI transformations applies what it calls a 10-20-70 rule: roughly 10% of success comes from the algorithm, 20% from technical infrastructure, and 70% from organisational design and change management. The technology problems are real but bounded. The people and process problems are open-ended, and most companies haven't assigned anyone to solve them.

A pilot that succeeds in a bubble proves the technology works. It proves nothing about whether your organisation will actually use it.

This is where implementation gets confused with deployment. Deployment is pushing the model to a server. Implementation is the harder job: redesigning the workflow it sits inside, retraining the people whose jobs it changes, and rebuilding the incentive structures that currently reward doing things the old way. Those three things take longer than any pilot, and they almost never appear on the project plan.

Ask yourself who loses status, autonomy, or performance credit if the AI tool works well. In most organisations, that list is longer than anyone admits. A team lead whose value came from knowing things that others didn't. An analyst whose throughput justified their headcount. A manager whose team is measured on process compliance rather than outcomes. None of these people are obstructing the pilot out of malice. They're responding rationally to the incentive structures they're in.

The final stretch between "working AI" and "used AI" runs directly through the parts of the organisation where change is most politically expensive. Most AI implementation plans simply don't address that stretch.

One practical test: look at how the KPIs of the teams using the new tool will change once it's fully adopted. If those KPIs stay the same but the tool makes the job easier, you'll see moderate uptake. If the KPIs change in ways that reward better outputs rather than more inputs, you'll see real adoption. If the KPIs don't change at all and the tool just adds steps to the existing workflow, the tool will quietly die. Map this before you build the rollout plan, not after.

Most AI implementation teams are built for the pilot phase: data scientists, ML engineers, maybe a product manager. The skills that matter most for the production phase are change management, process redesign, and user communication. These skills are rarely on the team, and when they're brought in late, they're treated as communications work rather than structural work.

User adoption is the single most underinvested area in enterprise AI rollouts. This is why tools designed specifically to smooth the onboarding and adoption phase, like user adoption and implementation tools, exist as a distinct category. The gap between "employees have access to the tool" and "employees are using it effectively" is where most implementations quietly stall.

The Gartner prediction that over 40% of agentic AI projects will be cancelled by 2027 is partly a cost story, but it's also a change story. Projects get cancelled when sponsors lose patience, and sponsors lose patience when they can't see adoption. You don't get adoption by deploying software. You get it by redesigning the work around the software.

The question isn't whether your employees can use the AI tool. It's whether their working day is now designed around using it.

What to Actually Do Differently

Three things, in order. Start before the pilot ends.

  • Appoint an implementation owner who isn't the tech lead. This person owns the workflow redesign, the change communication, and the incentive audit. They report to a business sponsor, not the CTO. If this role doesn't exist on your project, the production phase has no owner.
  • Run an incentive audit on the target teams. Before rollout, map how performance is currently measured for everyone who will use the tool. Identify where AI adoption conflicts with existing metrics. Either change the metrics or change the rollout plan. Doing neither and hoping for the best is the most common mistake.
  • Set a 90-day adoption target with a named metric. Not "the tool is live." Not "training has been completed." A number: percentage of target workflows running through the tool, time saved per user, error rate reduction. If you can't name a metric before launch, you're not ready to launch.

The technology is, at this point, good enough for most use cases companies are piloting. The constraint is almost always organisational. Treat the implementation as the hard part, staff it accordingly, and you'll find the 5% success rate starts to look less like a ceiling and more like a baseline.
